  • The first visit is going to be a comprehensive full-body musculoskeletal physical therapy evaluation. You and the therapist will discuss your history, your concerns, and your goals.

    She will explain more about physical therapy and the examination, and will try to include treatment on that first visit as well.

    The examination will include an assessment of your spinal/joint mobility and range of motion, muscle function, posture, balance, and soft tissue mobility. Your therapist will clearly explain everything beforehand. That way you know what to expect, and you can ask any questions you might have.

  • If you are coming in for a condition that affects your pelvic function (such as bladder, bowel, sexual conditions, or pelvic pain), your evaluation may include a pelvic floor muscle evaluation. Anatomical models and images will be used to demonstrate pelvic anatomy.

    The physical therapists at Link PT have advanced training in pelvic health physical therapy. Pelvic Health PTs can evaluate the muscle function (strength, endurance, timing/coordination, flexibility), pain, and soft tissue mobility of your pelvic floor, as this may affect your symptoms.

    For adults, this exam may include an external and/or internal physical examination of the pelvic floor. You may discuss any questions or concerns you may have about this examination with your therapist at your first visit.

  • Prior to your appointment, you will receive an email giving you instructions on how to fill out our intake forms, outcome measures, and consent forms. They are available here as well.

    Please complete them at least 48 hours prior to your appointment to hold your reserved time. If you have difficulty with accessing these forms, please call us promptly, so we can assist you.

    For in-clinic evaluations, it is best to wear comfortable clothes that are easy to move around and stretch in. Bring your photo ID, insurance card, and physical therapy referral, if applicable. Please arrive about 5 minutes prior to your appointment time to allow for registration.

  • In general, most patients are seen once a week, decreasing to every other week, depending on individual learning needs and rate of symptom improvement.


    After completion of the initial evaluation, your therapist can provide you with a better idea of treatment duration and frequency, based on your individual needs and goals.

  • We require 24 hours prior notice of cancellations. That allows us time to contact other patients who are on the waiting list for that day and will effectively shorten the length of time patients have to wait for an appointment.
    Our standard cancellation fee is $145 for reasons other than emergencies and illnesses.

  • There is no need to cancel your PT visit while you are on your period! We can see you during this time, unless you have an issue with it. If it is an issue, consider this when you schedule your appointments.

Visits

  • While most insurances cover at least a portion of physical therapy, it is recommended that you check with your specific carrier for details.

    Link PT is out-of-network with all insurances.

    We ask that payment ($180 per session) be made at the time of service (credit card, cash, or check). As a courtesy, we can submit the claim for you, unless you wish to do so yourself. You will be reimbursed directly by your insurer, based on your plan.

  • Because Link Physical Therapy is exclusively out-of-network practice, we are unable to provide Medicare beneficiaries with physical therapy services that are covered by Medicare.

    However, we can provide services that are not covered by Medicare, including wellness services and physical therapy for sexual dysfunction.

  • Link Physical Therapy will submit your claim for you after each visit, or, if you prefer, provide you with a detailed invoice for self submission.

  • All appointments (evaluations and treatment sessions) are $180. Payment is made at the time of service.
    Each appointment is a private, one-on-one session of about an hour.

  • The traditional insurance model limits us, as providers, from giving the best possible care. The out-of-network provider structure allows us to provide you with patient focused, quality based, and highly personalized care.

    With the out-of-network provider model, we are to be able to provide optimal care through:

    • Undivided, one-on-one sessions with licensed physical therapists (no therapy aides provide care)

    • A higher standard of care that meets YOUR individual health and wellness needs that are not limited by restrictions imposed by your insurance provider.

    • Rather than waiting 2 months or longer to be scheduled start PT, you will be scheduled promptly with a seasoned specialist that remains your dedicated therapist throughout the course of treatment.

    • Clinicians can focus on what they do best – provide excellent care for the whole person.

    We believe this patient-focused model results in better outcomes, utilizing your time and investment more effectively and efficiently.

    As a courtesy, we can submit your insurance claims on your behalf or provide you with a superbill, so you can submit your own claims. Many patients choose to utilize their health savings account (HSA) for sessions.

Insurance and Payment

  • We no longer conducting formal COVID pre-screenings prior to each appointment. However, we do ask that all patients and visitors refrain from coming into Link PT with any cold/flu symptoms and/or after testing positive for COVID-19 until both of the following are true, following CDC guidelines:

    For at least 24 Hrs:

    • Your symptoms are getting better overall, and

    • You have not had a fever (and are not using fever-reducing medication).

    All members of the Link PT staff will also adhere to these requirements .

  • Face masks are optional for patients/visitors and Link PT staff. Link PT staff reserves to right to require patients/visitors to wear a mask, if clinically appropriate during a session. Patients may request that masks be worn during their PT session, if preferred. Our Link PT staff are fully vaccinated against COVID-19.

  • We are limiting the amount of people in the clinic. Please inform us if you plan to bring a visitor, including children, in the clinic during your session. Please talk to your therapist with any questions or concerns.

  • Telehealth sessions are still available for all patients. They may be utilized in addition to or in replacement of in-person sessions. You and your therapist will go over the plan of care that will best suit your health needs while keeping everyone as safe as possible. More information on telehealth sessions is available below.

COVID-19

  • After your initial evaluation (in-clinic), you and your PT can discuss the options of telehealth vs in-person visits based on your individual needs and situation.
    What if your PT says you can do either in-clinic OR telehealth virtual visits? Some may choose to do a hybrid of telehealth and in-person visits, depending on the focus of each session. Your PT can discuss these options and help you weigh this out. We will work with each person to make it work for your own personal situation.

  • Yes! You and your therapist will go over the plan of care that will best suit your health needs.

  • Our PTs are licensed to practice in the state of Illinois, so patients must be physically located in Illinois during the time of their PT session.
